Download, print and bring the completed forms to your Coaches Meeting along with a copy of a photo ID and your signed "Coaches Code of Conduct" agreement. 3v3 Hoops on Fire Liability Release- MANDATORY Liability Release Form. This form is mandatory to participate in GDYBL's "Hoops on Fire" 3v3 Tournament. Please print off and give to your TEAM CAPTAIN. This form can be used for minors and adult participants.
Coaches' Code of Conduct *- GDYBL has adopted a Coaches Code of Conduct. We ask that you pledge to be responsible for your words and actions while conducting all basketball events and endorse all the following guidelines. Coaches are asked to please submit your signed agreement along with your CORI form to your age group coordinator at the mandatory coaches' meeting.
Parent/Player Code of Conduct- GDYBL has worked to adopt a Sport Parent/Player Code of Conduct. We ask that you pledge to be responsible for your words and actions while attending all basketball events and shall endorse the parent/player code of conduct.
Travel Coach Application- If you are interested in coaching a Travel Team, please fill out this form and return it to Sue Wynn, President Groton Dunstable Travel Basketball, PO Box 261, Groton, MA 01450.
